In my experience, publishers are looking for some way to get their hands
on the text for typesetting, and they need a general idea as to where
the images go.  They are used to receiving "Word" files which provide
the text and the author's idea of formatting, including emphasized words
and placement of images.  It is useful for them to be able to get their
hands on a "Word" file, but since Word doesn't render the same
everywhere it is sometimes also useful to send a "pdf" file.

If you create a pdf file using a PostScript driver and ps2pdf, the
publisher may not be able to extract the text for typesetting.

Depending on how much specialized text formatting is in the book, I
would suggest submitting a plain-text file, an OpenDocument file, a Word
compatible file, and the image files.  If a "pdf" cannot be created, the
publisher will likely be able to get what he needs for typesetting from
one of the other formats.
